To ensure services are tailored to your child's unique strengths, needs, and goals, all families complete an intake form prior to the initial consultation. These forms help gather information about your child's educational background, current supports, areas of strength, and family priorities. Depending on the services requested, families will complete one of the following: 

Academic Tutoring & Enrichment Intake Form

Autism, ADHD & Skill Generalization Intake Form

Homeschool Evaluation Intake Form.

All families are also asked to review and sign the Client Policies & Consent Agreement prior to beginning services. This allows us to establish clear expectations, maintain effective communication, and create the best experience for your child and family.
